Effect of nursing on postoperative respiratory function and mental health of lung cancer patients.

Both pulmonary rehabilitation training and psychological care have been shown to have a positive effect on the postoperative recovery of patients with lung cancer. However, few studies have combined the two to explore their combined effect. Therefore, this study aimed to investigate the effects of pulmonary rehabilitation training combined with psychological care on postoperative respiratory function and mental health in lung cancer patients. To investigate effect of nursing on postoperative respiratory function and mental health of lung cancer patients. 122 cases of lung cancer patients who underwent surgical treatment in our hospital and were treated in our department from January 2022 to April 2023 were selected and randomly divided into the control group and observation group. The control group performed the routine care intervention. The observation group was given pulmonary rehabilitation training and psychological care based on conventional nursing interventions. Forced expiratory volume, forced vital capacity. Maximum ventilatory volume (MVV) in one second was measured, and the patient's 6-min walking distance and dyspnoea index scale were used to assess the patient's respiratory condition. The Connor-Davidson resilience scale (CD-RISC), self-rating anxiety scale (SAS), and self-rating depression scale (SDS) were used to evaluate the mental health of the patients. There was no difference between the two groups regarding age, gender, education level, surgical procedure, type of pathology, and treatment (P > 0.05). After treatment, MVV, 6-min walking distance, toughness, strength, optimism, and total CD-RISC scores were significantly higher in the observation group (P < 0.05), dyspnoea scores, SAS, and SDS scores were substantially lower in the control group compared to the observation group (P < 0.05). Pulmonary rehabilitation training combined with psychological care for patients after lung cancer resection could improve lung function, enhance daily activities, effectively relieve negative emotions such as anxiety and depression, and reduce complications.

Psychological care of patients during the pancreas transplantation process: issues and prospects.

Pancreas transplantation for patients with type 1 diabetes is a therapeutic option when other treatments are not effective and physical complications occur. Psychological burden is prominent in patients, and non-adherence to treatment is often one manifestation of such difficulties. Time projection is an important factor affected by chronic disease. The prospect of transplantation has the potential to repair this disruption. It could re-establish a continuity in the patient's self and history, by connecting the future to a life that was only about past and present. Taking care of oneself, adhering to treatment, being part of a long-term therapeutic project and going through transplantation are all processes that need a good ability to self-project in time. This is specifically a domain of psychotherapeutic interventions. In this article, the psychological implications of pancreas transplantation for patients and caregivers alike will be discussed, as well as the role of the psychiatrist in the transplantation process.

An Introduction to Home Care for Esophageal Cancer

Esophageal cancer is a common tumor of the digestive tract and is a malignant tumor which affect one’s health and is potentially life threatening. The incidence and mortality rate of esophageal cancer in China is relatively high, with the incidence rate being the sixth highest and the mortality rate being the fourth highest among all malignant tumors. The lesion often occurs in the middle segment of the epithelium of the esophagus, often showing a family tendency, and is more likely to occur in middle-aged and elderly men. The typical symptom of esophageal cancer is progressive dysphagia, first with dry food, then with semi-liquid food, and finally with water and saliva. Treatment is divided into surgical treatment, radiotherapy, chemotherapy, and combination therapy. Apart from malnutrition and pain, the treatment can also cause great psychological stress to patients with esophageal cancer. Therefore, diet, pain, and psychological care for patients with esophageal cancer are crucial. Therefore, it more care is given to the patients by medical and nursing staff during hospitalization. However, not much have been discussed about home care for patients with esophageal cancer. In this article, we will talk about the home care of esophageal cancer patients from the following aspects: dietary, pain management, and psychological care.

Internet-based cognitive behavioral therapy for improving health-related quality of life in patients with endometriosis: study protocol for a randomized controlled trial

BackgroundThe trial investigates the efficacy of internet-based cognitive behavioral therapy (iCBT) in improving health-related QoL in patients with endometriosis, which is a chronic gynecological condition affecting up to 15% of people with female-assigned reproductive organs. Endometriosis is stress-related and comes with various physical symptoms such as pelvic pain and infertility. It has a substantial impact on health-related quality of life (QoL), and mind-body interventions seem promising in reducing the psychological burden.MethodsThis is a monocentric randomized-controlled trial recruiting 120 patients with endometriosis. The intervention consists of eight iCBT modules focusing on psychoeducation, cognitive restructuring, pacing, and emotion regulation. Participants will receive written feedback from a trained therapist weekly. The comparator is a waitlist control group. All participants will be followed up 3 months after the intervention, and the intervention group will additionally be followed up 12 months after the intervention. Trial participants will not be blinded to the allocated trial arm. Primary outcome measures are endometriosis-related QoL, pain, and pain-related disability. Secondary outcomes include coping, illness representations, and psychological flexibility. Statistical analyses will be performed following intention-to-treat principles.DiscussionThis randomized-controlled trial is the first trial to test the efficacy of iCBT for improving endometriosis-related QoL. Potential predictor variables and key mechanisms in treatment will be investigated to enable further progression in medical and psychological care for patients with endometriosis.Trial registrationClinicalTrials.gov, NCT05098444 Registered on October 28, 2021

Relevance. The global pandemic caused by the prevalence of COVID-19 (SARS-Cov-2) has become a trigger for the formation of a wide range of problems in the medical, socio-psychological, and rehabilitation spheres. The frequency of neurological or psychiatric diagnosis in patients after SARS-Cov-2, in the next 6 months, was 33.62%; the frequency of first diagnosis is 25.79%, and the mental health disorders themselves are diverse and affect almost all aspects of mental functioning, significantly worsening the quality of life. In addition, the extreme burden of the healthcare system in Ukraine in the context of a prolonged socio-economic and political crisis requires immediate реформування reform. Therefore, it is important to study the peculiarities of mental health of Ukrainian citizens in the context of epidemic danger and reform of the healthcare system at the present stage. Goal. To study the peculiarities of mental health of Ukrainian citizens in the paradigm of reforming the healthcare system in conditions of epidemic danger at the present stage. Materials and methods. Subject to informed consent in compliance with the principles of Bioethics and deontology, for the period from March to September 2021, 80 people aged 20 to 80 years were examined, who suffered covid-19 in 2020-2021 in mild (Group 1) and moderate severity – Group 2 (lung damage from 25 to 60%) and did not previously have a psychiatric diagnosis. The clinical and psychopathological method and screening diagnostics were implemented in the form of a semi-structured psychopathological interview, the main purpose of which was a comprehensive assessment of the patient's mental status. The psychodiagnostic method was used using the hospital scale of anxiety and depression HADS; the quality of life questionnaire SF-36; Being Activity Mood (BAM); The scale of C. D. Spielberger – Yu.L. Khanin. Results. The most common mental and behavioral disorders (MBD) using the HADS method are analyzed. The results showed that clinically expressed anxiety occurred in 33.7% of respondents (groups 1 and 2), and its subclinical variant – in 6.3%. The dominant plot of anxiety was phobic thoughts, fears of severe or fatal consequences of a viral disease in yourself or your loved ones. Clinically expressed depression was observed in 29.7%, and subclinically – in 68%. Assessment of the quality of life according to the SF-36 questionnaire revealed clinical indicators of the average score on the "vital activity" scale – 32.0 and 43.0 in groups 1-2, respectively. According to the BAM methodology, the self-assessment of the individual indicated a low assessment of all indicators in both groups. 75% of patients on the C. D. Spielberger–Yu.L. Khanin scale had high levels of situational and personal anxiety, and 25% had moderate levels. The level of situational anxiety was 56.70±0.48, and personal anxiety – 43.86±0.34 points. The Law of Ukraine "On rehabilitation in the field of healthcare " and the resolution of the Cabinet of Ministers of Ukraine "Issues of Organization of rehabilitation in the field of healthcare" are legislative and executive innovations of the present, which differ: pronounced" patient-centricity"; stages; multi-level; multicomponent influences; a significant range of rehabilitation coverage; interdisciplinarity; consistency; organizational and informational integrity; management efficiency; modernity, innovation. That is, the implementation of this law, if the necessary funding is available, will provide rehabilitation assistance to the population of Ukraine, including in conditions of epidemic danger, at the proper level. Conclusions. Thus, according to our study, as a result of the covid-19 viral disease, there was an increase in the number of patients with MBD: adaptive, anxiety-phobic and depressive. Organizational and methodological aspects of comprehensive medical and psychological care for patients with MBD in the framework of the development of the corresponding algorithm included: unity of measures, manageability of impacts; integrality; holistic approach; continuity, etc. However, an algorithm should be developed and brought into line with the presented innovations of healthcare reform in Ukraine for the terminology and formation of rational, effective directed routing of patients with anxiety-phobic disorders in conditions of epidemic danger in order to improve and increase the level of medical and psychological care and improve their quality of life. The Effects of Whole-course Psychological Care in Patients having Parkinson’s Disease with Anxiety

Objective: This research explored the clinical effects of the whole course of psychological care in patients having Parkinson’s disease with anxiety. Methods: Eighty-four patients with Parkinson’s anxiety disorder at Hubei No. 3 People’s Hospital from January 2020 to December 2020 were randomly selected for this research. They were divided into a control group and a study group using random number table. The control group was provided with routine care while the study group was provided with psychological care in which observation and comparison of clinical effects were caried out. Results: There were no significant differences in their scores based on the self-rating anxiety scale (SAS) and self-rating depression scale (SDS) between the two groups of patients before nursing, P &gt; 0.05. However, the scores of both, SAS and SDS of the study group after nursing intervention were lower than those of the control group. Sleep quality scores, patient satisfaction, and quality of life scores were all higher than the control group, P &lt; 0.05. Conclusion: The whole course of psychological care can effectively improve anxiety and depression of patients with Parkinson’s anxiety and improve their sleep quality.

COVID-19: Patient care after discharge from the Intensive Care Unit.

IntroductionThe Ibn Rochd CHU is a tertiary care structure that provides care for the most severe cases of COVID‐19 requiring hospitalisation in intensive care. The objective of study is to describe the complementary medical and psychological care of patients with COVID‐19 in the endocrinology department after a stay in intensive care.Patients and MethodsThis is a descriptive observational study of patients transferred from the intensive care unit to the endocrinology service following a COVID‐19 infection during the period from 17 April 2020 to May 26, 2020. Clinical characteristics of the patients and complications related to COVID‐19 infection were studied; a nutritional assessment using the MNA nutritional status assessment questionnaire; psychological assessment using quality‐of‐life questionnaires (Hamilton depression and anxiety, HAD, SF36, PCLS); a treatment satisfaction questionnaire (TQCMII) and an assessment of patient autonomy by the ADL score.ResultOur study included 41 patients with an average age of 55 years (19‐85 years), a sex ratio M/F of 1.05, 43.9% were diabetic, 34.1% hypertensive, 4.9% asthmatic and 5% obese, and 51.2% were severe and critical cases. The average ICU stay is 8.42 days, requiring intubation in 12.2% of cases. All patients were treated with the Hydroxychloroquine, Azithromycin, vitamin C, zinc and corticosteroid protocol, 14.6% had undernutrition and 65.9% had a risk of undernutrition. The average BMI was 25.34 kg/m2 (17‐42), 61% had experienced weight loss, which was greater than 8 kg in 26.1% of cases, 12.2% of patients were not autonomous, 12.2% had moderate depression, 2.4% severe depression, 14.6% mild to moderate anxiety, 12.2% severe anxiety and 29.3% suffered acute post‐traumatic stress disorder.ConclusionPatients with COVID‐19 are, in addition to the complications from coronavirus infection, vulnerable to undernutrition, psychological and motor complications. Additional care before discharge is essential for better integration of patients into their families.

Perioperative management of wounds with nanosilver dressing after neoadjuvant chemotherapy for breast cancer

Neoadjuvant chemotherapy (NAC) can improve the success rate of breast cancer surgery; however, most patients have obvious negative thoughts about NAC. Failure to effectively relieve these negative emotions or thoughts will adversely affect the quality of life of the patients after treatment. Therefore, a majority of nursing staff are committed toward reducing patients’ negative emotions through good perioperative management as well as improving the treatment effects and postoperative quality of life of patients. A good perioperative management of breast cancer should include postoperative wound and patient psychological care. This study found that patients using nanosilver dressings have greatly reduced total wound drainage, drainage tube removal time, and postoperative complications within four days after surgery and that the rate of wound healing was obviously improved. This suggests that nanosilver dressings can improve the rate of wound healing and reduce the risk of complications. At the same time, we found that postoperative negative emotions of patients receiving comprehensive care were effectively relieved and their mental flexibility was also improved. In addition, the quality of sleep, quality of life, and nursing satisfaction of patients receiving comprehensive care were significantly higher than those receiving traditional care models. This shows that the comprehensive care model is capable of reducing the negative emotions of patients during the perioperative period and also helps to improve the quality of life of patients after surgery. Therefore, this study demonstrates that the use of nanosilver dressings as a comprehensive care model can reduce the risk of negative emotions and complications in addition to improving the rate of wound healing and postoperative quality of life.

Interdisciplinary neuropalliative care: A unique and valuable clinical training experience for geropsychology trainees

ABSTRACT To address workforce issues in professional geropsychology, clinical training sites must offer opportunities to build skills for working with older adults and aging families. Neuropalliative care (NPC) may offer a valuable learning environment for geropsychology trainees to develop professional competencies, while positively contributing to patient care. This article describes a novel clinical rotation for an advanced geropsychology trainee in an interdisciplinary specialty NPC clinic. A co-assessment model emerged as a useful strategy for integrating the trainee into the established NPC team. Two case examples illustrate the co-assessment’s ability to enhance: collaboration within the clinic; psychological care for patients and care partners; and opportunities for the trainee to build competencies related to assessment, intervention, teams, and consultation. This paper concludes with a discussion of the benefits of NPC as a clinical training rotation for geropsychology trainees, as well as practical considerations for implementation in other clinics.

The Quality of Life, Depression Levels and Coping Styles of Patients on Kidney Transplant Waiting List

ABSTRACT This study aims to understand the coping strategies, depression levels, sociodemographic characteristics of Turkish patients with hemodialysis, and the other factors explain quality of life levels. This research is a cross-sectional study. The convenience sampling consisted of 292 patients with hemodialysis. The data were collected using the brief COPE, QOL, Beck Depression Inventory and sociodemographic form. According to the research results, patients show mild (score range 10–16) depression symptoms. One of the most important findings of this study is that the coping strategies and level of depression with chronic kidney patients accounted for a significant amount of variance in their quality of life. The relationship between emotion-focused coping strategy and quality of life was not statistically significant. In addition, high level of depression in chronic renal failure patients significantly reduce the quality of life. As a result of the study, the relationship between the high quality of life of patients who use problem-focused coping strategy more intensively and patients with low depression levels was determined. The findings point to the importance of taking individual coping strategies into account when evaluating the impact of a disease on psychosocial wellbeing. Delineation of coping strategies might be useful for identifying patients in need of particular counseling and support. These patients should have frequent contact with a multidisciplinary team of health care givers, possibly allowing for early diagnosis and prompt treatment. Results suggest targeted interventions to stimulate future research regarding the psychological care of patients with chronic kidney disease.

Psychologické aspekty a možnosti psychologických intervencií u pacientov s nešpecifickými zápalovými ochoreniami čreva

Psychological aspects of inflammatory bowel diseases (IBD) were given less attention in the social space of former Czechoslovak countries. The possibilities of psychological care for patients with IBD have to be re-evaluated due to the new worldwide trends in the management of IBD. In the complex management of IBD treatment arises a demand after psychologist as a helping member of the gastroenterological team in order to be available for patients, as well as to consult the cases with the specialists. In the following article we described psychological interventions (psychotherapeutic techniques), whose effectivity as a supplemental treatment in patients with IBD was examined. Recently, several studies on the effectivity of psychological interventions in patients with IBD have already been available. This trend is in accordance with a bio­psychosocial model of patients‘ care. Promising results were observed in psychotherapeutic techniques which use the work with body feelings, relieving the tension and relaxation training. The presence of psychologist and psychological guidance in difficult periods during chronic diseases such as IBD appears to be helpful. Patients with a psychiatric comorbidity tend to have worse course of disease. It is important to identify the patients who are in need of psychological care, mostly the ones with trauma anamnesis. Overcoming traumatic experience may provide positive impact on the disease course.

[The effectiveness of complex medical and psychological rehabilitation of women in the menopausal period].

Study of the complex medical methods' effectiveness and psychological rehabilitation of patients during menopause with the use of psychophysiological programs and certain psychotherapy techniques. The study included 90 women in menopause. The average age of the patients was 55.0±7.4 years. All patients were divided into 3 groups. In the 1st (30 menopause patients, average age 53.0±4.4 years) and 2nd (30 menopause patients with cancer or osteoporosis, average age 57.0±5.15 years) complex medical and psychological rehabilitation, including, in addition to drug therapy, physiotherapy procedures, manual therapy, psychophysiological and psychotherapeutic procedures were used. In the 3rd control group (30 patients, mean age 56.0±9.6 years), drug therapy, physiotherapy procedures and manual therapy were used. The analysis of the obtained data revealed the emotional disorders in patients of the 1st and 2nd groups before rehabilitation. There were low and decreased values of the subjective comfort index, SCI (31.4±5.12-40.7±5.21 points); high degree of fatigue reflecting a pronounced and strong degree of chronic fatigue, CF (27.00±3.53-41.40±6.12 points), high values of the personal anxiety indicator, PA (47.6±3.52-55, 1±5.91 points).After a complex medical and psychological care in patients of the 1st and 2nd groups the indicators characterizing the emotional and personal sphere improved significantly (p<0.05). The trial aimed to study the effectiveness of the complex medical and psychological rehabilitation of women during menopause showed a change in the indicators of the emotional and personal sphere after psychophysiological and psychotherapeutic influence, an improvement in the emotional state of patients and the life quality.

764-P: Art Therapy as a Novel Mental Health Therapy Aimed at Reducing the Psychological Burden of Living with Type 1 Diabetes

Objectives: Type 1 diabetes (T1D) is strongly linked with an increase in stress, depression, and anxiety. Mental health care is widely acknowledged to be an essential component of T1D clinical care. However, it has proven challenging to identify a successful model of mental health care for T1D. We investigated whether art therapy could be used in the psychological care of patients with T1D. Art therapy allows for the expression and exploration of complex emotions related to illness in an age-appropriate manner, and has been shown to be an effective mental health therapy in other chronic illnesses, but has not been studied in T1D. Methods: We evaluated the effectiveness of an art therapy intervention for stress management in T1D patients. We administered a 17-question questionnaire to 22 sequential patients referred to the on-site art therapist. Mean age of the subjects was 13.1 +/- 7.5 years (range: 8-40 years) and mean duration of diabetes was 5.3 +/- 5 years. The subjects saw the art therapist a mean of 7 +/- 6.4 visits prior to completing the questionnaire. Results: All participants found the sessions stress reducing and planned to continue with ongoing therapy and to incorporate strategies learned into diabetes self-care. The most common responses associated with art therapy were “calm,” “safe,” “feeling understood,” “less overwhelmed.” The average A1c prior to receiving art therapy was 8.7+/-1.8%, and fell to 7.7% +/- 1.5% after a mean of 7 art therapy sessions. Conclusions: Art therapy was a well-received effective mental health treatment in reducing stress in T1D. In this sample, glucose control improved as well. Art therapy is a novel modality in the psychological care of patients living with T1D that can alleviate some of the psychological burden of the disease, a treatment that will be studied in a larger trial. Significance of Psychological Nursing in oncology Internal Medicine

Abstract:Objective: To study and analyze the effect of using psychological care in patients with oncology. Methods: 66 patients with oncology internal medicine were collected and their psychological care was strengthened. The negative emotion score and quality of life score were observed and compared before and after nursing. Results: Patients had better negative emotional scores after care than before care; The scores of quality of life were significantly higher after nursing than before nursing (P &lt;0.05). Conclusion: In the nursing process of oncology patients, strengthening psychological care can significantly improve the negative mood of patients, so that they can maintain good psychological condition and improve their quality of life, which is worth popularizing.

Psychological Impact on Maxillofacial Trauma Patients - An Observational Study.

Facial disfigurement due to trauma is very common as the face is a prominent part of the body and is susceptible to injuries. A protocol for the diagnosis and intervention for psychological problems of trauma patients should be implemented in the Indian hospitals as they lack policies for assessing the mental status of such patients. This study was conducted to analyze and determine the psychological implications and need for mental health services of maxillofacial trauma patients. Fifty patients with maxillofacial trauma above 18 years of age were included in this study. The assessment was done using the Hospital Anxiety and Depression Scale (HADS) and Trauma Screening Questionnaire (TSQ), which are considered as the standard tools for assessment of psychological disorders post-trauma. We observed psychological stress in 84% of the patients at the baseline, which reduced to 24% after one month at the first follow-up visit and further reduced to 22% at the second follow-up visit. The reduction in the percentage from the first to the second follow-up visit was less significant. However, the change in HADS and TSQ scores was found to be significant, suggesting that the intensity of trauma decreases with the time-lapse. Anxiety, depression, and post-traumatic stress are prevalent in patients with maxillofacial trauma, which may lead to impaired day-to-day life. Our results suggest that there is a need for psychological care in patients with maxillofacial trauma.

[Psychological care in German intensive care units : Results of asurvey among the members of the German Interdisciplinary Association for Intensive Care and Emergency Medicine].

Psychological care should be provided in intensive care units (ICUs) because of the proven mental symptoms of patients and relatives. Even physicians and nurses can benefit from acorresponding care structure. Knowledge is lacking whether and how psychological care for patients and relatives as well as support for staff in German ICUs is implemented. For this reason, asurvey was conducted among the members of the German Interdisciplinary Association for Intensive Care and Emergency Medicine (DIVI) to gain an overview of the current structures and the need for psychological support. The members of DIVI were invited to participate in aweb-based survey. Atotal of 226 physicians and nurses took part in the survey. Analysis included statistics and group comparisons with Χ2 methods. In all care areas, psychological care of patients, relatives, and support for staff, respondents indicated asignificant undersupply and expressed the need for improved care. Amodel which provides consular or team-integrated support based on the level of care is conceivable. The current state of psychological care in German ICUs does not cover the existing need. Consequently the development of concepts and the beginning of discussions on how appropriate psychological care can be implemented in the future is necessary.

Effects of psychological care in patients with endometriosis: A systematic review protocol.

Background:This systematic review will be proposed for investigating the effects of psychological care (PC) in patients with endometriosis.Methods:We will search the following 7 electronic databases from inception to the present: MEDLINE, EMBASE, SinoMed, Web of Science, Cochrane Library, Chinese Biomedical Literature Database, and China National Knowledge Infrastructure. We will include randomized controlled trials for evaluating the effects of PC in patients with endometriosis. Cochrane risk of bias tool will be used to evaluate the methodological quality for each included study. Two authors will independently carry out the study selection, data extraction, and methodological quality evaluation. Any disagreements will be solved by a third author through discussion.Results:This proposed systematic review will use high-quality evidence-based medicine to evaluate the efficacy and safety of PC for endometriosis. The primary outcomes include depression and anxiety. The secondary outcomes consist of pain intensity, health-related quality of life, and adverse events.Conclusion:The findings of this study will provide convincing evidence to determine whether PC therapy is an effective and safe intervention for patients with endometriosis.PROSPERO registration number:PROSPERO CRD42019123292.
